Job Description
**Responsibilities**
+ Social Work Assessments
+ Facilitate patient/family conferences with the rehab team as needed
+ Educate patient/family of the rehabilitation program
+ Advocate for patients to obtain ARU authorization extensions, address insurance denials, and facilitate insurance appeals
+ Assist with discharge planning (ordering DME, HH/OP referrals), etc.
+ Assist in post acute placement on complex discharges and engagement of appropriate agencies of community resources
**Qualifications**
+ Requires California MSW license
+ LCSW preferred
+ Three (3) years clinical experience in acute rehabilitation and/or medical/surgical

$40.66 - $58.96 /hour
